Patient Documents
1. Plasma Request form (Just request the hospital to prepare ILBS plasma forms, they will make it for you, its their job)
2. Blood samples of patient (again just tell the hospital)
3. Aadhar Photocopy of Patient
4. Aadhar Photocopy of Attendant
5. Thermacol box
Donor document
1. Aadhar photocopy + take original with u
2. Positive report photocopy
Donor Eligibility
1. Atleast 28 days from Date of Positive Report and upto 3 Months.
2. No prior surgery, Major Desease. Female donor shud not have been pregnant ever.
3. Shud not be on Antibiotics or any strong medication.
Procedure
First obtain the donor registration from New Building Ground Floor. Donor fills first two pages of it, then register it at the Registration Desk Counter 4, located just in adjacent Building. Then donor goes to Second Floor New Bulding where they take his blood sample and test it.
Now, if donor blood is fit and has required Anti Bodies, he will be notified on phone. Some of reasons of rejections; low/high BP, low heamoglobin, No Anti Bodies, etc. If your Donor gets rejected, then arrange another donor and repeat the same process until your donor is approved. It usually takes 1.5 to 2hrs to get donor sample tested. But sometimes, if there are more number of donors, then this could take 3-4hrs.
Now upon calling, the donor donates the blood and he is given a slip and refreshment. The attendant with his Plasma Request form registers at the registration desk. The donation slip along with Blood Samples is to be taken to Blood Bank located at Old Building Basement -1. The attendant gives blood sample of patient and donation slip in the window of Blood Bank, and then gets plasma which is to be kept in a thermacol Box.

Any donor blood group can donate and in return any blood group plasma can be obtained.
Thermacol Box can arranged from hospital itself or can be bought from a Medical Store. A medical store named "MediMart" located outside ILBS sells these box and also ice cubes.
Ice cubes is not mandatory. If you're travelling long distance like 2-3hrs, then keeping plasma along with ice cubes is suggested."
